Allergy Specialists of Knoxville, PLLC is a provider for most major insurance plans. Please call our office prior to your visit to make sure that we are in network with your insurance plan. We will file your insurance claims for you if you have provided us with your current insurance information. To ensure claims are processed under the current policy, we will ask to see and/or copy your insurance card at each visit. We are also required to collect your co-payment at the time of service. You are responsible for charges that are not covered by your insurance. For your convenience we accept cash, checks, MasterCard and Visa. Our office staff will be happy to assist you in determining insurance acceptance. Please call us at 865.588.2753.

Questions about benefits, such as coverage for skin testing, blood testing or allergy shots, should be directed to your insurance company or employer.

We ask that each new patient complete a number of forms at the initial visit. For your convenience, we will mail those forms to you prior to your visit or you may print the forms below. Please complete all the forms and bring them with you to your first visit. This will help us get you ready to see the doctor more efficiently.

Please stop all antihistamine medications (prescription and over-the-counter) 7 days prior to your initial visit only. Please DO NOT stop any of your other medications. If you are not sure which of your current medications are antihistamines, please call us. We will be happy to review your medication list with you. On the day of your appointment (new and returning patients) please bring:

  1. Your insurance card
  2. Physician referral forms, if required by insurance
  3. List of prescriptions and/or over-the-counter medication, including dose and frequency

For new patients only:

  1. Please bring a cardigan style (button-front) shirt, sweatshirt or sweater AND a sleeveless shirt
  2. Please allow 2 – 3 hours for your initial appointment if you are being tested

Please request that your current physicians send us:

  1. Information and updates about your medical and surgical history
  2. Recent x-rays, CT scans and any other testing that has been done for you

Please send all records and correspondence to the main office (1346 Dowell Springs Boulevard, Knoxville, TN 37909) even if you are being seen at one of our satellite offices. You may also fax your records to the main office. Our fax number is 865.588.7418.
