Welcome to Allergy Specialists of Knoxville
We are dedicated to providing high quality personalized care for patients with allergies, asthma, sinus disease, and immunodeficiency. This appointment time has been reserved especially for you. New patients should allow two or three hours for the first appointment.
If you are unable to keep this appointment please let us know at least 48 hours in advance so we can make the slot available to someone else.
We ask that each new patient complete a number of forms at the initial visit. For your convenience the following choices are available:
1) We will mail those forms to you prior to your visit- please complete all forms and bring them with you to your first visit
2) You may print the forms here– please complete all forms and bring them with you to your first visit
3) You may request a link (which will be emailed to you) to complete the forms online through our secure patient portal.
Things To Know
- PLEASE STOP ALL ANTIHISTAMINE MEDICATIONS (prescriptions and over the counter) for 7 days prior to your visit.
- Please click on the following link for a list of antihistamine medications that should not be taken prior to your visit.
- DO NOT stop any of your other medication. Please click on the following link for a list of common medications that do not interfere with allergy testing. Please call us if you are not sure which of your medications are antihistamines. We will be happy to review your medication list with you.
- Please request that your current physician(s) send us information and updates about medical and surgical history, recent x-rays, CT scans and other testing that has been done for you.
- Click on the link to complete the release of medical records form to request your records from another physician.
- Records may be faxed to 865-588-7418 or mailed to our main office at Allergy Specialists of Knoxville, 1346 Dowell Springs Blvd, Knoxville, TN 37909. Please have records sent to the main office even if you are being seen at one of our satellite office.
On The Day Of Your Appointment
- Bring a list of your prescriptions and any over the counter medications, including dose and frequency.
- Wear or bring a cardigan style (button-front) shirt, sweatshirt or sweater and a sleeveless shirt. DO NOT wear one piece clothing.
- Please bring your most current insurance card and a photo ID to your visit.
We are in network providers for most insurance plans. If you need more information about your plan, please contact our office prior to your appointment. If your plan requires a referral please bring that referral with you or fax it in advance to 865-588-7418.
For your convenience, we accept cash, checks, VISA, MasterCard and Discover. We are required to collect your co-payment at the time of service. We will file your insurance claims for you. Any charges not covered by your insurance will become your responsibility. Our office staff will be happy to assist you in determining insurance acceptance.
